Be a U.S. citizen and a resident of Alabama
Meet certain medical criteria
Must be resident of an approved medical institution for at least 30 consecutive days
Have resources below a certain limit
Have a monthly income below a certain limit

Tips for Applying for Medicaid


When a loved one needs nursing home care, it can be very stressful. Due to many laws and regulations, the process of applying for this type of Medicaid requires patience and understanding. There are things you can do to make the process easier:

Check to see if your loved one qualifies for any benefits from other sources such as VA benefits.
Be aware Medicaid is required to "look back" 60 months or 5 years before the application date to see how income or resources were spent.
You must provide all documentation requested in a timely manner. Make copies of anything you need to keep.

Things that can make a Claimant Ineligible


If the claimant's countable resources exceed $2,000 on the first day of the month or any time thereafter during the month, the claimant will be ineligible for Medicaid for that month and will have to private pay.

Transfers of assets/resources/property may affect eligibility. For the institutionalized claimant the transfer of assets/resources/property by claimant or claimant's spouse can cause the claimant to be ineligible for nursing home payment. Medicaid can look back 5 years.

Information Needed for Medicaid Application


These are some items that you will be required to provide that will be sent to Medicaid with your application:

The following cards are required for verification purpose: Medicare, Medicaid, any other health insurance cards, Medicare part D drug, Social Security Card, & ID.

Statements from each bank account for 5 years (including cancelled checks).

Copies of any annuities, CDs, etc.

Copy of deeds to all property. (Joint, individual, heir, or Life Estate.)

Copies of deeds of any property sold or otherwise disposed of in the past five years.

Insurance premium verification for Medicare Part D or supplemental medical insurance.

Copy of Power of Attorney, Guardianship, etc. if applicable.

SAVE proof of gross income from Pension Company, VA, Social Security, and any other income sources. (This cannot be verified from bank statement only, must have proof     from source.)

Copy of tag receipts for all vehicles.

All Life Insurance Policies for both resident and spouse. Policy needs to show the face value and cash value.

If you have done pre-arrangements with a funeral home, please provide an itemized copy for what items were purchased and total amount.

Name and social security number of current spouse and any previous marriages, as well as beginning and ending dates of all marriages.

Medicaid may require other information as requested.
